The Jordan Health Foundation provides financial support for the Anthony L. Jordan Health Corporation which operates 9 Federally Qualified Health Centers in our region.
All locations offer primary care services for medically underserved and uninsured patients, along with sliding fee scale discounts to keep care affordable.
Your support, when given through the Jordan Foundation, will help ensure Jordan Health patients remain healthy, strong, and vibrant members of our community.

Join Us For Impact
Your gift will help close the gap for youth and families in need of preventative or urgent care. Consider making a legacy gift that will safeguard healthcare delivery for our neediest neighbors for years to come. Giving is easy; you can make your gift today in person, online, or by mail.
Urgent Care
Open to the public at 82 Holland Street with evening and weekend care, staff attends to acute injury or illness for people who don’t have a doctor (or whose doctor is not available) and want to avoid a long wait in an overcrowded ER.
Access To Care
Basic medical/dental suites at three public housing facilities bring care “closer to home” for residents, neighbors and teens. Your donation can be used to help increase much-needed staffing, or help purchase new medical supplies.
Facility Upgrades
Capital improvements within our three comprehensive health centers are a necessity to ensure we continue to deliver quality care to our patients. Please consider making a gift to help maintain and upgrade facilities that are in constant use by those who most need care.

Leave A Lasting Legacy
Supporting Jordan Health patients with a planned gift creates a legacy of care that increases in breadth with the passage of time.
A planned gift is a way for a donor to leave a gift of money or other assets to a charity or nonprofit.
This most commonly occurs after the death of the donor and is bequeathed in the form of a life insurance policy, property, or through a will. Please consider making a planned gift. Your donation will leave a lasting legacy which will ensure that Jordan Health has the resources to provide quality care for future generations of patients in the Rochester region.

Gifts of any level make a difference:
A gift of any size is tax-deductible, and will help provide care for patients at any of the 9 Jordan Health Centers across the region. See how your dollars work to deliver the services that meet the needs for many members of our community:
$600
Covers the cost of a full series of prenatal visits for one birth.
$200
Will provide a Jordan Care Card for any adult, child, or expectant mother for a complete primarycare visit including basic blood work and prescription discount.
$100
A gift of $100 can provide a mental health evaluation.
$75
At the $75 level, your gift will cover the cost of an annual OB/GYN exam.
$50
A donation of $50 will underwrite a dental cleaning (adult or child), a sports or college physical, a routine eye exam, or a sick child visit.
$35
Your contributions go right to work providing basic care for families and individuals. A contribution of $35 will support the cost of an oral examination to help keep our patients smiling.
